A good overview of the approach longevity clinics are taking. Most of us here have worked are way up the pyramid and are at, or near, the top, I suspect.

I’m struggling a bit with whether the bottom two sections of the pyramid should be reversed?

I rationalized the sequence by thinking that most people are sick in some way: a1c / insulin resistance, BP, heart disease, kidney function, etc. the first thing to do is to figure out what is going to kill them first. Get drugs to dramatically slow the accelerated aging. Then get the life in order; maybe get off drugs someday.

I say to always go for low hanging fruit. Quick wins provide motivation. It’s hard when the only things in life that provide pleasure have to go. It’s not a good life but it’s all they have.

They didn’t mention peptides?

Also the supposedly atherogenic 2py/4py thing comes from NAD, not directly niacin. But you hear people getting good things from even the IV NAD. So this one is not (for me) very solid. It has a ways to go.

That certainly makes sense from the standpoint of extending the average lifespan of the population. I don’t know that it’s best for extending health span?

In my mind, they should be reversed from pragmatic perspective. Not all have access to comprehensive diagnostics but all can attempt lifestyle changes.
