Very high serum creatinine

My serum creatine came back 1.69 from the stable 1.27 it has been 2 months ago…, is this always bad?

what do you suspect?

medications -

telmisartan 80
acabose 50 - one tablet
rapamycin 5mg weekly
simvastatin 40
ezetimibe 10
jardiance 25

started taking a lot of glycine with my coffee…

started smoking a bit more - about 6-7 cigs a day only though

supplements

novos core (vitamin c/magnesium/calcium/glucosamine sulfate/rhodiola rosea/l theanine/hyaluronic acid/fisetin/ginger/pterostilbene/lithium)

vitamin d/k2/magnesium/beef liver/melatonin

I’ve read some medications raise serum creatinine without harming the kidneys - is this telmisartan?

I’m reading diuretics can cause this - taking jardiance with telmisartan together a bad idea?

Im reading taking creatine can increase serum creatinine (glycine a component of creatine)?

All of them affect kidneys. Are you watching your potassium (telmisartan 80 may elevate it to unhealthy levels)?

I’m thinking its causing hypotension which in turn is affecting…

I’d also like to say I took this test in a third world country, will retest when I get back to the states

Are you on these medications for specific diseases or for longevity?

longevity, but both. I would take an ARB anyway for CKD but a lighter dose. AND sglt2 inhibitors are prescribed for CKD. And statins are thought to help CKD I read

I went telmisartan 80 because I thought I read studies of high dose helping CKD more

I actually did an initial serum creatinine test with all the meds while starting two months ago and it was fine, so hopefully this is the glycine causing non damaging temporary spikes

Creatine is often increased in athletes.

Cystatin C Test:** This is an alternative to creatinine for estimating glomerular filtration rate. Cystatin C is less affected by muscle mass than creatinine, making it potentially more accurate in athletes.

I’ve read some stuff that in CKD proteinuria could be more important than serum creatinine, but thats against current guidelines, and that telmisartan could possibly raise serum creatinine while decreasing proteinuria in some circumstances? I’ll be getting a battery of tests to get to the bottom

I am concerned though that the combination of high telmisartan dose (80) and high sglt2 inhibitor dose (jardiance 25) in combo is causing too much of a diuretic affect, making the kidneys work harder, even though both substances on their own are though to aid CKD/kidneys

You can also consider Urea and Urate to give an indication as to kidney function (these are also driven by other things).

Its worth doing a second creatinine test before seeing a nephrologist. There are plenty of places in the UK you can get same day results on that. This should be the same in other countries.

My money would be on the Jardiance causing it. I started on 12.5mg and ran blood test after 4-6 weeks. Creatinine went from usual 65-75 (UK measurements) to 95. After reducing Jardiance to 6.25mg it was back to 86 three weeks later and 79 five weeks later. Normal UK range is 59-104.
I also discovered that I was drinking nothing like the amount of water I should have been (darker urine with Jardiance)
Otherwise I take very similar meds to you.

Interesting. I heard jardiance can cause it.

Issue is I was taking that for half a year before the telmisartan/glycine and some recent herbs ive been ingesting without issue…

Ah! Perhaps not the Jardiance then in your case. Shame as it would’ve been an easy one to adjust dosage with.

One thing you said is for sure, and this is the problem we in the longevity field playing our own doctor face, I wasn’t drinking enough water at all dealing with these agents acting as diuretics.

It can be the jardiance in combo with the telmisartan is dehydrating too much. And I drink a lot of coffee which causes diuretic action too

Retest creatinine and test Cystatin C.

Some factors: unusually high amount of protein before the test, lifting heavy the day before, or taking creatine.

glycine is a component of creatine, and I was doing grams of it… that might not go with those who have CKD… when I retest I will be off.
