Just got my LabCorp test back. Blood drawn 1 hr 45 min after taking, with a tablespoon of Udo’s Oil. 6mg Rapacan from an Indian pharmacy. Result was 16, so it looks like it’s the real deal. I take that once a week, have done so for probably 3 years.
Sirolimus Level Measurement and Dosing Strategy
I recently measured my Sirolimus level at UCK Gdansk, yielding a result of 2.4 ng/ml. This measurement was taken exactly 6 days after ingesting 3 mg of Rapacan (sourced from India) under the following conditions:
- Consumed one grapefruit 3 hours before dosing
- Took a shot of fish oil with the dose
- Followed up with a shot of olive oil 45 minutes later
The detection range for this test is 2-30 ng/ml. Given that “levels should drop below 1 ng/ml within 24-48 hours before the next dose” and considering the half-life of Sirolimus (62-80+ hours), I’ve calculated my optimal dosing interval:
- ~80 hours (3.3 days) for levels to drop to ~1 ng/ml
- Additional 1-2 days before the next dose
Total interval: 11 days between doses (6 + 3 + 2)I plan to maintain this dosing strategy. Any feedback or suggestions would be appreciated.
Previous Peak Level Test
In a prior test, I measured my peak Sirolimus level after taking 2 mg Rapacan:
- Result: 9.3 μg/l (equivalent to ng/ml)
- Timing: One hour after intake
- Conditions: Without grapefruit
Proposed Future Testing
Given that the upper detection limit is 30 ng/ml, taking 3 mg Rapacan with grapefruit would likely exceed this limit. Instead, I’m considering the following test:
- Dose: 1 or 2 mg Rapacan
- Conditions: With grapefruit and oils as previous test
- Timing: One hour after intake as previous test to compare
This approach could help estimate the multiplication factor of grapefruit on Sirolimus levels. My hypothesis is that it could be >3 and if so I should be taking only 1 mg. Would you recommend this testing approach or suggest alternative methods?
Upper detection of 30? That is low. The labs here in Australia can detect much higher than that. Anyway, personally I think you need a higher dosage if you need to fix something. For maintenance, I take 10mg fortnightly with GFJ in line with a 85 hour blood test of over 4ng/mL. However, I feel that 20mg fortnightly with GFJ is better … but it is a huge spike in the first 2 hours… I took that dose weekly for nearly 6 weeks… and problems seems to clear up very fast. I am going to take 2x 10mg dose over the next 2x fortnights before ramping it up again.
I just realized Marek offered this test. It’s not that expensive either.
Curiosity is probably now going to get the best of me when it comes to testing the bioavailability of novel rapamycin RoA’s (IM, IR, IN). Does anyone have information regarding a non-toxic and effective solvent for IM injection? I’m thinking propylene glycol mixed with water might work.

I’m going to be doing a series of personal dosing studies, where I do the Marek blood sirolimus test at the 2 to 3 hour time period, then the 24 hour, then 48, 72, etc hour time periods to get a feel for my personal rapamycin AUC and curve. I’m also doing at the same time the Healthspan Starter Longevity Blood panel to track the impact (if any) on my key blood measures: Longevity Starter Panel
I’m going to start at 10mg/week and go up to 20mg per week and see what the effect is. Will report back as I complete it.
Are you going to take the Healthspan panel at each of these time points also? That’s quite a bit of testing, but the information would be very interesting. I think I’ll follow your lead on that.
Do you know any Telehealth providers offering the below test? Would provide information on how rapa effects the adaptive immune system.
Yes - perhaps not all, but all of the tests initially, then if numbers are not moving much I’d relax the testing schedule. Its only $40 for that healthspan test, so not too bad.

To be honest Nick… several on here like me literally take the rapamycin with the grapefruit juice simultaneously… I squeeze one fresh red grapefruit and get about 5 fluid ounces.
I get a 3 times increase 2.5 to 3 hours later with a LabCorp test.
I will run this protocol again December 4th for a trough first and C-max after dose…GFJ and 2.5 hour wait. Will share results here.
Most people here figure out their own dosing protocol based on their body’s response to rapamycin (any side effects at any given level?) - after starting low (e.g. 1mg/week) and working up from there.
Some people here have mentioned following Bryan Johnson’s protocol for a while, but Bryan recently announced he’s stopping rapamycin, see the thread here: Bryan Johnson stops rapamycin
Bryan doesn’t provide much detail on the test results / rationale behind his actions generally, but I think he was trying to balance out the higher dose (in theory more benefit at least at the levels we are typically dosing), and the long half-life of rapamycin.
On the timing of GFJ and rapamycin, I think most people take the GFJ a few hours prior to dosing and seem to get a good response, based on sirolimus blood level tests. You can also do this to test your results.
It’s not 6 hours after ingestion. The maximum inhibition occurs from 0-4 hours after ingestion. That’s why it works well to take the rapamycin with or shortly after drinking the GFJ. As for the multiplier effect. It’s not 3x, it’s anything fro 0-8x. The average is just around 3.5x that’s why people refer to that. But in reality it can be far from that. That’s a major reason blood tests are necessary.
Lifeextension has stopped selling this test. Actually since maybe Nov '24. They refunded my un used tests just recently. Any other vendor? I like to occationally test since I’m a low absorber and have to take >>6mg to get to 20ng/ml (???). 20 is just a number I’ve personaly seen a ffew times. I may have seen high 20’s and alot of very low, 2hr post taking.

Actually Curt… 3 ng/mL is pretty average per 1 mg rapamycin.
Taking 6 mg and getting 20 ng/mL is great. That is the range of Matt Kaeberlein from his Optispan podcast 20 - 21 ng/mL.

For me to get to 20ng/ml (??) I need to pre-dose with freeze dried graphfruit powder (Fresh GF is now available here in Atlanta butt last year some thing happend and nothing, not even frozen juice. So I added the other boosters; berberine, peperine, maybe grapseed extract. 2hrs before THEN 15mg ish might get me 20ng… I’ve gotten lazy. Compliance is dropping for hop skip jump complications. I need to re test my new dose needs are given laziness and just one morning vita-mixy-kitchen-sink. I have Rapa coming out of my ears so upping dose till seen in the blood is my logical path… ;(

I’ve been VERY frustrated with the state of affairs for womens health, especially all 4 hormnone replacement; NP thyroid, 17a-estradial, oral pregesterone, testosterone (maybe topical). I have 3 sisters not on any HRT >>60yr and as i search for local clinics I’m finding few per visit charging places. Many have gone concierege, $3k one go. Then the mail order folks have sprung up like this gethealthspan, offer testing and product at a flat rate at about 3x what we buy for and who knows how little they pay. For my sisters a complete 4 hormone HRT would be well over >>$500/mo well over. That is crazy how men and women are getting screwed by Alopathica, now the Funcional clinics with concierge then the mail order places sweeping up who fall through face to face solutions.
Us DIYers who buy from India etc havee no idea how rotten “the system” really is. Not untill I dug into BHRT for my sisters did I see how bad going above board is.
I’ve been searching to buy the 4 hormones no-prescreiption. Its hard to get what you want. NP-thyroid is mfgered at such a low volume there’s little inventory outside the US prescription system so zero desicated thyroid (T4, T3, T2, T1) outside the US. Estrodial can be bought, topical, but testosterone is both expensive and hard to find without a prescription… The weight lifters use injected, 100mg/1ml via a carrier oil, 1 injection / week. I divide in half and inject 2x a week to level out DHT, SHBG, take 15mg boron too.
