What is the acarbose/canagliflozin for fats?

Something that blocks absorption of fats, or makes you excrete them out when you have a nut binge, or at minimum reduces whatever negative impact they have on your inflammation markers/blood vessel integrity. Ideally without diarrhea

There was Olestra, but I think it got banned here.

Wow, @bicep you nailed it. Olestra even produced some of the same gastro issues as acarbose :wink:

Is orlistat different from olestra? I think orlistat might still be for sale?

Chitosan was pretty hot in this market for a while. But it’s ability to bind fat has been pretty much debunked? even though it has reduced some fat related markers and seems to help with weight loss. Dosing is probably the key, as is often the case.

Conclusion

Yes, it lowers LDL about the same as a moderate intensity statin. That is expected to give a good life/healthspan boost.

NEXLIZET is a combination of 2 medicines, bempedoic acid and ezetimibe.

This is the reason I’m probably not going to try either of those, either on their own or in a combo. My daughter is “floxed” and has suffered greatly due to fluoroquinolones, which are black-box warning antibiotics and should only be used as a last resort. It’s been 6 years of hell for her. You do not ever want to be floxed.

NEXLIZET and NEXLETOL can cause serious side effects, including:

  • increased levels of uric acid in the blood, which can lead to gout, a painful joint condition. Call your doctor if you have any of the following symptoms: severe foot pain especially in the toe joint, warm joints, swelling, tender joints or joint redness.
  • tendon rupture or injury. Tendon problems can happen in people who take bempedoic acid, one of the medicines in NEXLIZET or NEXLETOL. The risk of getting tendon problems while you take NEXLIZET or NEXLETOL is higher if you: are over 60 years of age, are taking steroids, are taking antibiotics called fluoroquinolones, have renal failure, or have had tendon problems in the past.

But what has that got to do with bempadoic acid and ezetimibe?

You are much more likely to die of heart disease than uric acid / tendon injury, so my bias is to lower the risk of the major killers and not dwell too much on low risk scenarios.

I wonder what the percent of people (using bempadoic acid or ezetimibe) are impacted by these two conditions? I track my uric acid (and its too low right now) - so not an issue, and tendon injury seems like a pretty minor risk for reducing the risk of cardiovascular death, the number 1 killer of people.

Since you can track uric acid levels and moderate if necessary, the only real issue is this one, and I view it as pretty minor for healthy people with no history of tendon ruptures:

TENDON RUPTURE

In clinical trials tendon rupture occurred in 0.5% of patients treated with bempedoic acid vs. 0% of placebo treated patients and involved the rotator cuff (the shoulder), biceps tendon, or Achilles tendon (package insert). Tendon rupture occurred within weeks to months of starting bempedoic acid and occurred more frequently in patients over 60 years of age, in those taking corticosteroid or fluoroquinolone drugs, in patients with renal failure, and in patients with previous tendon disorders. In the CLEAR Outcomes trial tendon rupture was similar in the bempedoic acid and placebo group (bempedoic acid 1.2% and placebo 0.9%) (239).

While the risk is very low, the result is devastating if fluoroquinolones are part of the equation. These are commonly and mistakenly prescribed for infections that could be dealt with by safer antibiotics. So I’d suggest that if one decides to use NEXELIZET or bempedoic acid to be aware of this one aspect of risk.

The main culprit is fluoroquinolones and it appears from the warning that NEXLIZET may increase the risk of getting floxed if you take fluoroquinolones.

A tendon rupture is just the beginning of being floxed. It goes on and on with a wide variety of other issues. There is no cure, it can put you in a wheel chair with debilitating pain for the rest of your life.

And it can kill you but many have committed suicide to stop the relentless pain.
