Sirolimus blood test

Hi,

I received my lab result for a Sirolimus blood test. Test was exactly one hour after taking 10 mg. The previous dose had been one week prior.

The main reason for taking the test is to make sure that the medication I order from India is real rapamycin.

My level came to 3.3 ng/ml. This is lower than what I was expecting. Perhaps I needed more time to reach peak.

My question: does this result suggest nonetheless that the medication I am taking is adequate quality rapamycin?

Hi and welcome to the forums.

That seems a little low (the 3.3ng/ml number) but its in the “reasonable” range given the typical Tmax timing of around 0.7 to 0.9 hours (with a typical standard deviation of about .2 hours). You did your testing at 1 hour which means the level was falling rapidly at that time. Getting peak levels is like catching a falling knife… very hard (as you can see by the very sharp peak and rapid fall in the graph below).

What brand was it?

I take Sirolimus. I thought it was low as well. The lab’s website says that the peak occurs within a 2-3 window, so I thought I had taken the test too early. But it sounds from what you are saying that I may actually have taken it too late.

Typically I do my test 2 to 2.5 hours after the dose.

You could have tested too early.

Per Pfizer, it takes an average of three hours to reach Tmax after taking their pills on an empty stomach. Up to 2.3 hours for Rapacan. And other brands are unknown. The information that Rapadmin posted above is for Sirolimus liquid suspension.

Thank you for correcting my mistake. I missed that:

Oral nonaqueous solutions containing 5 mg/mL siro- limus or placebo were supplied. Before administration, the sirolimus concentrate or placebo was diluted with distilled water by an unblinded third-party dispenser who did not participate in screening, drug administration, or the evaluation of any subject.
