Increlex (Mecasermin) is a recombinant human insulin-like growth factor-1 (rhIGF-1) indicated for the long-term treatment of growth failure in children. It is specifically designed for patients with Severe Primary Insulin-like Growth Factor-1 Deficiency (SPIGFD) or those with growth hormone (GH) gene deletion who have developed neutralizing antibodies to GH.
Clinical Details
- Mechanism of Action: Increlex acts as a replacement for endogenous IGF-1, which is the principal mediator of growth hormone’s effects on bone and tissue growth. In patients with SPIGFD, the body produces GH but cannot effectively generate IGF-1.
-
Indications: * Pediatric patients (≥2 years old) with height and basal IGF-1 standard deviation (SD) scores \le -3.0.
- Growth hormone gene deletion with neutralizing antibodies to GH.
- Contraindications: Active or suspected neoplasia, closed epiphyses (growth plates), or known hypersensitivity to mecasermin.
- Administration: Subcutaneous injection. It is critical to administer Increlex shortly before or after (±20 minutes) a meal or snack due to its potent insulin-like hypoglycemic effects.
-
Dosage: * Starting dose: 0.04 to 0.08 mg/kg twice daily.
- Maximum dose: 0.12 mg/kg twice daily.
Pricing and Availability
Increlex is a high-cost specialty pharmaceutical. As of early 2026, pricing remains substantial, reflecting its orphan drug status.
| Component | Estimated Cost (USD) | Notes |
|---|---|---|
| Wholesale Acquisition Cost (WAC) | ~$5,000 – $6,500 per vial | Per 40 mg/4 mL (10 mg/mL) multi-dose vial. |
| Price per mg | ~$125 – $160 | Costs vary by pharmacy and payer contract. |
| Annual Treatment Cost | $50,000 – $150,000+ | Highly dependent on the child’s weight and dosage titration. |
Note: Most patients access this drug through private insurance or Medicaid, which typically requires strict Prior Authorization ¶. Payers generally require proof of SPIGFD (Height SD $\le -3.0$) and confirmation that growth hormone deficiency has been ruled out via provocative testing.
Critical Safety Information & Side Effects
- Hypoglycemia: The most common and serious acute risk. Patients must have adequate caloric intake around the time of injection.
- Lymphoid Hypertrophy: Common enlargement of tonsils and adenoids; periodic examinations are required to monitor for sleep apnea or hearing loss.
- Intracranial Hypertension: Some patients may develop increased pressure in the head, manifesting as headaches, nausea, or papilledema.
- Slipped Capital Femoral Epiphysis (SCFE): Rapid growth can lead to hip or knee pain; any limp should be evaluated immediately.
- Malignancy Risk: There have been post-marketing reports of both benign and malignant neoplasia in children treated with Increlex.
