Increlex (Mecasermin) is a recombinant human insulin-like growth factor-1 (rhIGF-1) indicated for the long-term treatment of growth failure in children. It is specifically designed for patients with Severe Primary Insulin-like Growth Factor-1 Deficiency (SPIGFD) or those with growth hormone (GH) gene deletion who have developed neutralizing antibodies to GH.

Clinical Details

  • Mechanism of Action: Increlex acts as a replacement for endogenous IGF-1, which is the principal mediator of growth hormone’s effects on bone and tissue growth. In patients with SPIGFD, the body produces GH but cannot effectively generate IGF-1.
  • Indications: * Pediatric patients (≥2 years old) with height and basal IGF-1 standard deviation (SD) scores \le -3.0.
    • Growth hormone gene deletion with neutralizing antibodies to GH.
    • Contraindications: Active or suspected neoplasia, closed epiphyses (growth plates), or known hypersensitivity to mecasermin.
  • Administration: Subcutaneous injection. It is critical to administer Increlex shortly before or after (±20 minutes) a meal or snack due to its potent insulin-like hypoglycemic effects.
  • Dosage: * Starting dose: 0.04 to 0.08 mg/kg twice daily.
    • Maximum dose: 0.12 mg/kg twice daily.
