It has been shown previously that 14% weight loss due to a very-low-energy diet (VLED) resulted in counter-regulatory changes in appetite-regulating hormones, including a significant increase in fasting levels of ghrelin and significant reductions in the fasting levels of leptin, GLP-1, peptide YY (PYY), cholecystokinin (CCK) and amylin48. During 1 year of follow-up after the end of the VLED intervention, the direction of these changes remained, but only the increase in ghrelin levels and reduction in fasting PYY levels were significant
These actions are thought to be primarily mediated through central brain populations of GLP-1 receptors; for example, in the hypothalamus, semaglutide affects the pro-opiomelanocortin and cocaine- and amphetamine-regulated transcript (POMC/CART) and neuropeptide Y and Agouti-related protein (NPY/AGRP) neurons known to suppress appetite and stimulate food intake, respectively51
