GENERAL STUDIES ON GAUNFACINE:
Increased cognitive function with guanfacine when treating long COVID: “Guanfacine is a highly selective α2A-adrenergic receptor agonist that is thought to strengthen pre-frontal cortex connectivity and reduce neuro-inflammation and has been used to treat cognitive disorders such as attention deficit hyperactivity disorder (ADHD) (36). It is also thought that guanfacine would be effective as a treatment for cognitive deficits in long COVID (37). A single case report found that eight out of 12 reported improved cognitive abilities when they were with guanfacine (1 mg for the first month, increased to 2 mg after 1 month, if well-tolerated) and 600 mg N- acetylcysteine (NAC) daily (38). Four patients discontinued therapy, two for unspecified reasons and two due to hypotension and/or dizziness, common side effects of guanfacine. Because the use of Guanfacine remains restricted to specific indications and age groups in Australia (people aged 6–17 years as monotherapy for ADHD), there is limited access as a treatment for long COVID symptoms.”

Guanfacine reduced heavy alcohol consumption and multiple forms of cognitive function:
“Here, we show that the α2 AR agonist clonidine worsens ethanol-induced hypothermia and sedation in male mice, while the more selective α2 AR agonist guanfacine is devoid of these effects. We also observed that, in male and female mice, while both clonidine and guanfacine reduce heavy alcohol drinking, guanfacine does so with higher potency. Furthermore, guanfacine improved cognitive performance in a temporal order test and, partially, in a novel object recognition test but had no effect in a novel spatial location test, in male and female ethanol-experienced mice. Finally, we found that chronic intermittent ethanol drinking increases the number of persistently activated NE neurons in both the locus ceruleus and the nucleus of the tractus solitarius, in both male and female mice. Our results highlight a central role for the α2 AR system in heavy alcohol drinking and associated cognitive deficits, suggesting that α2 AR stimulation may represent a viable pharmacological strategy to treat AUD.”

Guanfacine may aid in improving sleep caused by amyloid beta induced disruption of Alzheimer’s disease: “5xFAD mice displayed dark phase-specific hyperarousal and impaired brain state transitions by 2 months of age. LC neurons exhibited increased tonic firing due to impaired Kv4 and Kv7 potassium channel conductance, resulting from soluble amyloid beta (Aβ)-induced disruption of α2A adrenergic receptor regulation. Pharmacological activation of α2A adrenergic receptors restored Kv4/7 function and normalized LC excitability. Local administration of guanfacine (α2A agonist) or retigabine (Kv7 modulator) significantly rescued sleep-wake disturbances.” Impaired adrenergic regulation of Kv channels underlies LC hyperactivity and early-onset sleep disruption in AD-like amyloidogenic mice pubmed.ncbi.nlm.nih.gov
Guanfacine shows unique benefits for people with sleep apnea: “Guanfacine, but not hydrochlorothiazide, improved baroreflex sensitivity, heart rate variability and flow mediated vascular dilation, suggesting that decreasing the elevated sympathetic nerve activity of obstructive sleep apnea returned vascular function toward normal.” Sympathetic overactivity and nocturnal diuresis in obstructive sleep apnea alter the response to hypertension therapy pubmed.ncbi.nlm.nih.gov
STUDIES ON GUANFACINE SPECIFICALLY PERTAINING TO ADHD (still may have relevance outside of ADHD):

2-methyldihydrexidine (2MDHX) + guanfacine (GFX) outperformed 2-methyldihydrexidine alone in increasing types of memory and cognitive function: “We found that when the selective full D1 agonist 2MDHX was administered concomitantly with a selective α2A agonist (either CBG or GFX), either temporal order memory or cognitive flexibility was improved in rats with deficits. MPH, on the other hand, did not improve temporal order memory, spatial WM, or cognitive flexibility.”
Neurotypical rats performed worse when dosed with 2-methyldihydrexidine alone or in combination with guanfacine but it might be a dose response issue: “The benefits of co-treatment, however, were tempered by the detrimental effects on rats that were performing well already. Co-administration of the selective D1 agonist 2MDHX with the selective α2A agonist CBG or GFX, not only significantly lowered cognitive flexibility, but also tended to impair temporal order memory. Similar detrimental effects were observed with monotherapies as well. When administered alone, 2MDHX significantly impaired both cognitive flexibility and temporal order memory. When either GFX or CBG was administered alone, there was significantly decreased cognitive flexibility or a trend to impair temporal order memory. One limitation of these studies was that the drug dose chosen might have been on the descending phase of the inverted-U dose response curves of these rats with superior baseline performance”
