What single change to the research effort on Alzheimer’s could make the greatest difference ?
Seems like gene modification from ApoE4 to ApoE2 should be on the table.
The FDA has approved several gene therapies for various conditions, including treatments for sickle cell disease, Duchenne muscular dystrophy, and certain types of cancer.
Envision a world where people are tested for Apo E type, and then treated with the Apo E gene therapy. What would the health care benefit be to saving millions of people from Alzheimer’s ?
Rather than focusing on relatively rare genetic diseases, why not put the money where it can make the greatest difference for society. Apo E4 is one of these. Candidates for other targets of the highest impact for gene therapy ?

So, now it has been a year since you started intra nasal insulin. My husband and I both also started last April. Other than may-be slight appetite reduction, have not noticed anything, but will continue.
A related tangent: my husband had been experiencing extreme fatigue. Along the way we tested for several things, discovered he has a mild MGUS that is unlikely to progress, that his testosterone is normal, blood work is normal (other than the light chains). But his fatigue persisted. He decided to stop taking all meds and supplements. None of them was that important, really. But his fatigue ended! So we will start adding back. Of everything he took, the lysine and saw palmetto were probably the most efficacious, as they ended his cold sores. In any case, I think it was probably the memantine that has been deviling him.
In other news, my husband had the blood test for AD, and the results, combined with the PET scan that assessed Tau, showed that the probability of his developing Alzheimers is very low. The MCI is real, but seems to be stable for now.
I no longer use nasal insulin. Subjectively, I felt zero effects, good or bad.
As for memantine, I have a year’s supply, which I will never use. I felt nothing but negative side effects from taking it.

A much better alternative, IMO, is Semax, N-Acetyl Semax, or Semax nasal spray. I have used N-Acetyl. It produces almost instantaneous results, though I don’t take it every day. Others in the forum have also noted good results from Selank. I sourced Semax powder from Nootropics Source and reconstituted it with bacteriostatic water and put it into my own nasal spray bottles from Amazon. These are gray market peptides that were originally studied in Russia. Peptides in general do not travel well in reconstituted form, so it is best to keep them in powder form and reconstitute them yourself. They are best kept in dark containers and refrigerated after reconstituting them. IMO: Never buy more than a month’s supply. The process of reconstituting them yourself is simple. The nasal spray dosage often used, for which there are no definitive studies, is 400-900 mcg daily. I used 1000 mcg 0.1ml daily because that is what my spray bottles dispense.
